Stop Payment Form

A stop payment form is required if you need to cancel a missing, lost, or stale dated cheque issued by the University of Toronto and recorded on your ACORN invoice.

The cheque referenced below, issued by the University of Toronto, is payable to me and has not been received, has been lost, or is now stale-dated.

By submitting this form, I request that the University cancel the cheque identified below and issue a replacement payment. I acknowledge that, if the original cheque is found, I will not attempt to cash it and will securely destroy any stale dated cheque in my possession.

I understand that processing a replacement payment may take approximately two to three weeks from the date this form is received. Processing time may be longer for cheques issued more than 3 years ago.

Note: complete a separate stop payment form for each cheque that you believe is missing, lost, or stale dated.
